Selective COX-2 Inhibitors: Towards Defining Their Appropriate Clinical Role

Nonsteroidal anti-inflammatory drugs (NSAIDs) are among the most commonly used therapeutic agents. With the introduction of the selective cyclooxygenase-2 (COX-2) inhibitors, the use of this category of medications has expanded; however, the appropriate place for these agents in the therapeutic armamentarium is yet to be defined. Selective COX-2 inhibitors are effective in treating both acute and chronic pain and inflammatory and noninflammatory arthritis. Due to their gastrointestinal safety, they have become the nonsteroidal drug of choice for many clinicians despite their higher cost compared to nonselective NSAIDs and limited long-term safety data with respect to gastrointestinal and cardiovascular end points. Large multicenter trials have demonstrated that the selective COX-2 inhibitors reduce the burden of ulceration compared with traditional NSAIDs [1,2]. However, even with their demonstrated protective gastrointestinal advantage, controversies have emerged and fundamental questions remain unanswered. This review will focus on the use of these agents, address the controversies regarding their adverse events, and discuss potential new uses for the coxibs as a class.
